If you submit the form via email, a copy of the completed form will be sent to the email address you indicate, for your records.

PDF, or Portable Document Files, will provide you with a hard copy of the original MARE form, which can then be filled out by hand and submitted to the MARE office via mail or fax (734) 528-1695.
